Good web hosts?

I'm in search of a good web host that has quality customer service and a long-standing tradition of not screwing up and being down for a month. I'm looking for the following features: 100MB or so of space Multiple POP3 accounts (with a catch-all system) SMTP PHP4 At least 2 MySQL databases, preferably more Perl/CGI Fast Ping Time/Response Time/Download Time/etc... Telnet Unlimited Bandwith (or at least high) Maybe multiple FTP accounts Maybe RealMedia Sub-domains Maybe ColdFusion I would like this to be around $20-$25 a month. Any recommendations are welcome. Thanks. BTW - The reason I need a new host is because my old host has been giving me no customer service and they've had my email and site offline for the past 2 days. It's $25/month on Unix Apache server with 500 MB of space.
